The temporomandibular joint (TMJ) is what connects your lower jaw to your skull, sitting just in front of each ear. Most joints just move one or two ways — this one hinges open and shut and slides forward, back, and side to side, sometimes all at the same time. That’s a lot to ask of one joint, and it’s part of why it’s so prone to trouble. Think about it: every time you talk, chew, or swallow, you’re putting this joint to work, thousands of times a day, without ever noticing.
When the joint, the muscles wrapped around it, or the little disc that cushions everything stop moving together the way they should, that’s what we call TMD (temporomandibular disorder), or TMJ syndrome. Here’s the thing a lot of people don’t expect — the actual problem often isn’t inside the joint at all. It’s in the muscles and connective tissue surrounding it. That’s a big reason treatment usually zeroes in on the jaw muscles just as much as the joint itself.
A handful of things tend to be behind TMJ problems:
Bite misalignment – When your upper and lower teeth don’t quite meet the way they should, that mismatch puts uneven pressure on the joint over time.
Jaw injury or trauma – A direct blow to the face or jaw can hurt the joint or the tissue around it.
Arthritis – Degenerative or inflammatory arthritis can break down the cartilage and bone in the joint, same as it would in any other joint in your body.
Chronic teeth grinding and clenching (bruxism) – Grinding your teeth, whether it’s at night or during a stressful week, keeps the joint and surrounding muscles under constant strain.
Displaced disc – That small cushion of cartilage between your jawbone and skull can slip out of place, which is usually what’s behind clicking, popping, or your jaw locking up.
Stress – Even people who’ve never ground their teeth in their life can end up unconsciously clenching their jaw when stressed, and that tension adds up just the same.
More often than not, TMJ disorder isn’t traced back to just one culprit — it’s usually a mix of a few of these. That’s exactly why getting a proper evaluation matters before diving into any kind of treatment.

Signs You Need TMJ Treatment
Here’s some reassurance: most patients can get their TMJ disorder under control without ever needing surgery. As a first step, we’ll usually recommend:
Occlusal splints (night guards):
Custom oral appliances that shift your jaw into a better position, cut down on clenching and grinding, and take pressure off the joint.
Medication:
Anti-inflammatories, muscle relaxants, or pain relievers to get you through flare-ups.
Trigger point injections:
These target specific spots of muscle spasm to relieve pain and take strain off the joint.
Lifestyle and stress management:
Figuring out what's actually triggering the clenching posture, habits, stress patterns and addressing that directly.
Physical therapy and jaw exercises:

Targeted stretching and strengthening that opens up your range of motion and eases muscle tension.
Botox injections:
This relaxes the overactive muscles behind all that clenching and grinding, easing pain and tension for months at a time. Because we're oral and maxillofacial surgeons, we know facial anatomy inside and out, which means precise, effective Botox placement.
Most patients feel real improvement from these conservative approaches on their own, often within just a few weeks to a few months.
If conservative treatment isn’t giving you enough relief, or there’s an actual structural issue inside the joint, surgery might come up as an option. These range from pretty minor to fairly involved, and we only ever consider them once conservative treatment has had a real chance to work:
A minimally invasive, in-office procedure that flushes out the joint to clear inflammatory buildup and get movement back, usually done under light sedation.
A tiny camera goes into the joint so your surgeon can actually see what’s happening and handle minor corrective work while in there.
This one’s reserved for more complex structural problems, where your surgeon needs to directly repair the joint and in select cases, replace it entirely.
Whatever your case looks like, your surgeon will always steer you toward the least invasive option that’s likely to actually work, and will walk you through the risks and realistic outcomes before anything moves forward.
So what does TMJ treatment cost actually run? It depends a lot on the approach. A custom night guard or splint tends to be the most affordable route. Botox and trigger point injections sit somewhere in the middle and are usually billed per session. Surgical procedures cost more the more involved they get — anywhere from a quick in-office arthrocentesis to more extensive joint surgery. The good news is most non-surgical treatments and diagnostic evaluations get at least partial coverage from dental or medical insurance. We’ll give you a clear cost estimate and help you make sense of your coverage after your evaluation.

Sometimes, if it’s mild and tied to a temporary stressful stretch or overuse. But if you’re dealing with persistent pain, locking, or limited jaw movement, that typically won’t just fade on its own it’s worth getting checked out before it becomes a long-term issue.
Really depends on what’s causing it and how severe it is. Some people notice a difference within days of starting a splint or Botox, while others need a few months of therapy and monitoring before they feel lasting relief.
Most people don’t, honestly. The overwhelming majority of TMJ cases get handled just fine with conservative treatments like splints, physical therapy, and Botox. Surgery only enters the conversation when those approaches genuinely aren’t cutting it.
